If you require only Hepatitis A, Diphtheria, Tetanus & Polio and Typhoid please make an appointment at the surgery at least 4 weeks prior to departure date.  You will need to complete this form prior to making your appointment.  Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

These clinics provide comprehensive travel advice and details of any additional vaccinations you may need to travel. We can provide you with your vaccination record to assist with this.
