Annual Reviews and Monitoring with Medlink

From 1 September 2026, the practice will be using a secure system called Medlink to help manage annual reviews and ongoing monitoring for patients with long-term health conditions and those taking medications that require regular checks.

Medlink is used by GP practices across the UK to help ensure that reviews, blood tests, health checks, and medication monitoring are completed at the right time. It allows patients to provide important health information in advance, helping our clinical team review their care more efficiently and identify any follow-up that may be needed.

How does it work?

You may receive an invitation by email or SMS text message asking you to complete a personalised online review. The questionnaire is tailored to your individual health conditions, medications, and monitoring requirements and can be completed on a smartphone, tablet, or computer at a time that suits you.

Once your review has been completed, you may be invited to book any follow-up appointments that are required, such as:

• Blood tests
• Blood pressure checks
• Medication monitoring
• Other long-term condition reviews

 If you do not start or complete your review, you will receive:

• One reminder after 48 hours.
• A further reminder 2 weeks later.

What happens next?

Once we have received your review responses and any test results, these will be reviewed by a clinician. We will then take the appropriate follow-up action, which may include:

• Confirming that your review is complete.
• Arranging further tests or monitoring.
• Reviewing your medication.
• Providing advice or information.
• Arranging a telephone or face-to-face appointment if needed.

I don't use email or a smartphone

If you do not use email or a smartphone, you may receive an automated telephone call inviting you to contact the practice to arrange your review in the usual way.

Do I have to use Medlink?

No. If you would prefer not to complete your review online, please contact the practice and we can arrange your review and monitoring in the usual way.
