Registration Process:

1.  Complete and submit this form.

2.  Check your email: One of our team will send a confirmation email in due course from an @nhs.scot address. 

3.  VITAL FOR ACCOUNT SET UP: REPLY TO OUR EMAIL TO RECEIVE FURTHER DETAILS REQUIRED TO SETUP YOUR ACCOUNT.

Patient Services currently operates as a manual process, meaning that once a registration form is completed and submitted, it does not automatically create or activate a patient account. Instead, the submitted form is reviewed and processed by staff, and additional steps are required by you to fully set up the account within the system. Patients should be aware that submission of this form alone does not offer immediate access to services, and you will be contacted for confirmation of your email as part of the setup process.

Eligibility Criteria:

• Patients aged 16 and over: Must possess an individual email address to comply with GDPR regulations.

• Parents registering children aged 12-15: If the child is capable of making their own decisions, they must have an individual email address. For younger children or those not deemed capable, a parent/carer email may be used.
